ATHLETICS DEPARTMENT MANDATORY FORMS
All Student Athletes must have the following forms completed prior to participating in any practice and/or game for Mercy Vocational High School:
Physical Exam (Pages 1 and 2 of the following document)
Family Health History Form (Page 3 of the following document)
There are no exceptions to this rule. Head coaches for each sport have their individual Permission Slips. Click on a specific sport on the menu on the left to find out who the Head Coach is for the sport.
